Have a look at RRDTool  (http://oss.oetiker.ch/rrdtool/)
This should provide the majority of what you're looking for.

I'm not sure how much it does of catching problems and notifying you
about them - but it's certainly worth considering for the data collection,
storage and analysis tasks.
   There are also a whole slew of tools that build upon it - one or more
of which might be what you're looking for.  Follow the "RRDWorld"
link on that website.
