	Exactlly like that?!  That is how you copied it below... *sigh*  You'll need to remove the brackets (which indicate optional parameters) and replace the SAMPLE parameters with theones that work in your environment:

	FROM:  sender of email (does not need to have an actual mailbox, just don't reply to these emails)
	SMTPSERVER:  your email server - you'll need to talk with your IT about this
	ADDRESSES:  who gets the email?
