If logging in on windows client the login dialog is automatically
changed to ask PIN only. However with rdesktop I'm still shown the
dialog asking for username and password.

I've compiled 1.6.0 with smart card support and this is what I'm getting:

merike@Mary-Jo:~/Töölaud/rdesktop-1.6.0$ /opt/bin/rdesktop -r scard

Are there any limitations concerning the type of card reader and/or
card affecting smart card logins?
