In the manpage you reference, I see:

>          F  flags (read/unread, `*' and `#' tags, header number)

so I'd guess those are the flags:

   - means unread message
   D means read message (will be deleted from the list later)

and a number if you mark that message with "#" and an asterisk if you 
mark the message with a "*".

I tried setting your header display format and I see lines like:

11  - [Lorenzo Thurman     ]:  6 Billing system [21 May 06 10:44  ]

I don't know what to do to see a percent sign instead of the hyphen.

 From the manpage you reference:

Maybe %B only does something if you're reading in offline mode?  I don't 
know for sure.
