Just for clarity:
I have at least the possibility to delete the whole domain to get rid of anything.
If i delete a selection of pages only, however, there's a chance to produce 'orphans'. So far this is all about the same domain, not about 'adds' from other hosts.

I guess that accordingly deleting a whole domain doesn't delete any dependent 'add' domains, which were called by the first domain only, accordingly ?
So there would be another source of orphans.

So keeping a cache clean works best with fast general expiring, or some amount of manual work.

Well, of course, you always said wwwoffled is no web arvhive...
