What year is it?

Can you trace the wires from the pigtail and see where they go? 

Have you contacted Mazda (the company or a local dealer may be able to pin
it down)?

Failing that find one in a junk yard or on the street (you'll want to talk
to the owner first going down this avenue) and see what it is. This will
give you a part number or in the case of the junk yard possibly a used
replacement part.
