The more dangerous concern is that there is backfeed from a parallel connected power source. I have seen older installations where there is more than one source from a single phase, possibly to avoid having to run a bigger wire. Not a good way to do things but it has happenned. Another possibility would be a malfunctioning power transfer switch if there is redundant power available. The transfer switch would have to have really good phase matching though, or the clouds of black smoke and noisy bang would give it away immediately :) That would explain the voltage drop too, if the alternate feed was inadequately sized. I know it is an inconvenient test for a NOC, but with equipment connected, if you open the breaker then AC voltage should measure in millivolts, if not actually zero.
