At a consulting client (which is my only exposure to SP-type activities 
these days), the client gets an MPLS VPN where we build all of their 
infrastructure.  They get one or more "egress points" where the VPN 
connects to "the Internet", and those are the points where the customer 
is metered or rate-limited.  I think the MPLS services are lumped into 
the customer's aggregate bill (they tend not to do line-item-type 
billing for their customers).

At a former employer, we never had anyone get an MPLS VPN and our 
Internet access, but we priced the MPLS VPN ports based on the port 
speed (T1, MetroE, etc.) and that handled the billing function.
