“filtering it out” on a blanket basis is just not an option.

Depending on a tip line and other activity that is tracked - e.g.: payments via shady and complicated routes, plus good old fashioned policing “that fellow is looking very strangely at my ten year old daughter in the pool" tends to be what nails the neighbourhood perverts much more often than any”web filtering” that is going to scale across an ISP.
