Graham I think you centered the problem.
using smbpasswd I set up an account:

andrea

the win box is configured in this way:
the computer name is andrea and the user is also andrea
all small caps

from my mac I can enter "andrea" computer
but when I try to enter in the server from andrea i get an error 
message even not a login prompt.
