Ok, apologies. This turns out to be user error. I just tested it with OpenBSD
and it worked fine. I noticed on OpenBSD that the carrier was active on the
other port (even though that port was unconfigured). I swapped the cable for
port 0 to the lower port and it worked.

I went back to FreeBSD and used the lower port for port 0 (instead of the upper
port) and it also works on FreeBSD but it never gives the carrier signal on the
unconfigured port like OpenBSD does.

This bug can be closed. Thanks to everyone who responded and for nudging me to
try other operating systems.
