An extreme example happened to xkeyb, which I inherited from Dietmar 
Höhmann, and which isn't commandline compatible to MS-KEYB. I have 
planned to change this some day, but in the meantime, at least I tried 
to submit an up-to-date version of the help file.
I guess it is easier if all the maintainers submit not only their LSMs, 
but also a brief HTML for help. After all, if I recall correctly, the 
HTML files are just quoted between the "literal" tag (that I can't 
remember).
Other posibilities would be that developers compromise to pack with the 
programs just one more file (like changes.txt, copying.txt,...) called 
syntax.txt or the like, where the syntax is briefly described, with 
notes if neccessary, and possibly examples (as in MS-DOS Help files), 
and related commands.
The last (and most difficulting) is for the HTML-Help maintainer to try 
* /? and guess the help, but I guess this is much harder.
