After some further testing and board-swapping, I've discovered that the
problem is due to a hardware fault on my Challenge S. I found that a
Phobos G130 also failed to work but with different symptoms. The card
was recognized as tlp0 but the kernel then said "sorry, unable to handle
your board". Two cards failing in the same machine was a bit suspicious,
especially since I knew that the Phobos card was working fine under 4.0.
Swapping the mezzanine card from another Challenge S solved the problem,
my Set card is now working.

Just for interest, I put in an Irix boot disk before I swapped the
mezzanine card and ran the SGI diagnostics. After running for over three
hours, they reported no fault found. Anyone know if the diagnostics
actually test the mezzanine card on the Challenge S?
