Not my dilemma, personally.

And who sets up the governing body, and how is it decided who sets it up, and
who is on it and who is not, and so on?

Because that itself is part of the governance.  The process has to start
somewhere, somehow, with some particular group of people (or individual).

Where, with whom and how does yours start?
