We need centralized documentation for the C
pre-processor defines used to affect the compilation of
Expat so people porting to new platforms will be able
to understand what each is for and how they relate to
each other.

Added documentation for the feature-configuration
pre-processor defines in doc/reference.html revision 1.48. 
The internal macros defined in lib/internal.h don't need to
be covered in the general documentation, and are described
in that header file.
