RE: rule context="X" semantic?

I believe it's more along the lines: a node can only be evaluated once in a
pattern. Therefore, if your "B" node was matched by a previous rule context,
it won't be taken into account rules following it inside the same pattern.

Correction: it's not an abnormal behavior but just a conform to the
specification.

This is the behavior of schematron in respect of patterns:
"
For each node and each pattern, the rules are evaluated in the order in
which they appear in the Schematron schema and this evaluation stops after
the first matching rule.
