No/wrong page numbers in ToC and index

Hi,

my ToC shows "XX" for everything (there's no "adult content" in
my document, however), the index shows page "2" for almost
everything, but the indexterms are on pages 2..50.

xmlroff is 0.6.0
libcairo2 is 1.8.6

What am I doing wrong?
