I will recommend to have a lok at GeeXbox :
http://www.geexbox.org/screenshots/ . I used this distro quite a long time
ago, possibly 10 years from now. The distro is 10 megabytes in size (newer
versions are a bit more heavy) and can boot from a CD or USB. I am not sure
if it is maintained anymore, but it was made specifically to make a very
old system to work as a media centre. It has a stripped down Linux kernel,
and its own UI. This is a very old review which I did once:

improved it a lot. I ran this distro on an Intel Pentium MMX processor with
less than one Ghz freq and 128 MB of RAM.

Else, for other smaller general purpose OS, you might want to give Puppy
Linux and CDLinux a try, but these does not focus to transform a system
into media centre, but these being very lightweight, runs very smoothly
even in a very old computer.
