ExtUtils::Instal and ExtUtils::MakeMaker long predate File::Spec.  The 
APIs are subtly different.  To upgrade them is, therefore, painful.

Not unless there's a specific reason to do so, no.

There haven't been any changes to File::Spec for Mac OS in some time.
