Well no one's responded, so I will:

Short answer:  there is no such thing.

Long answer: Have you ever tried using the "trace" utility on a full color
picture in Flash or Adobe Illustrator?  It doesn't work very well.  What you
are asking for is the same thing for music.  You are asking for a utility to
turn an apple into an orange.  Or more like a prune.  Or a date.

You want a program that can hear through all of the frequencies of a sound
recording to the actual original note , and I have yet to find a COMMERCIAL
product that does that well.  Cakewalk Pro Audio did it through version  8,
but it didn't do it well  and they dropped it for version 9.  No one missed
it.  I don't know about Pro Tools, but I don't have $9,000.00 to drop on a
piece of software. I also have a Boss DR-5 drum machine that will take a
guitar input and try to create Midi information from it.  It is TERRIBLE at
it .

Basically, the only way to do it reliably is manually - find each note on an
instrument and input it into a software program that can save it as a midi
file.  Hard and time-consuming.  The less you know about music notation and
theory the harder this will be.
