Probably you can't. If advj doesn't recognize them, probably it's a
problem of the SDL driver. SDL is the library used to interfacing with
the input device.

Ah! Ok, effectively this is a problem. It isn't obvious that you must
enable them manually. The problem is that if the joystick is enabled,
the MAME core ignores all the keyboard inputs. Maybe I will fix this
for the next version and will enable the joystick as default.

I tried also in Linux. The same problem exists. I suppose that it's a
problem of the game emulation. I checked and AdvanceMAME reports the
correct values (like adv)j at the MAME core.

This menu is used only in DOS. Another not obvious thing which need to be fixed.
