It's a standard web-application in terms of connectivity, so that's really a question for your network team.  

There's four basic approaches
# Create an internet pipe that allows you at home to tunnel into your work network and use the resources there (including Jira) as though you were in the office.  "Virtual Private Network" or VPN is the phrase to use, although there are variations.
# Create an extension of your work network at home.  When I started using computers, we used dial-up modems over 'phone lines, one phone-call and your computer was part of the work network, but there are much faster options nowadays (ISDN and it's faster relatives are the ones to look for now)
# Put your Jira on the internet, by hosting it outside your work network
# Expose your work network to the internet, at least allowing Jira to appear somewhere within your public-facing side.
