IVTV Patch Included At Last

I have gone over Martin Crossley’s ivtv patch, and included an adaptation of it in my copy of dvdauthor <http://github.com/ldo/dvdauthor>. This patch does 2 things: it creates places for filling in DVD-Video navigation information if the original video lacked them, and it deals with MPEG system headers occurring in certain places that are valid, but where the original code wasn’t expecting them.
