I have filled in all of the bits formerly marked here as undocumented
<http://wiki.github.com/ldo/dvdauthor/dvdauthor-xml-syntax>. This 
includes clarifying the use of the attributes on the <cell> tag, and the 
fact that <subpicture>s and their child <stream>s can be specified at 
both the pgcgroup and the PGC levels.
