Square holes are no problem - you just use MPB's built in "block" object type instead of cylinders. For other polygonal shapes I usually employ a set of overlapping blocks. (Or, especially for weird curved shapes, you could use the material-function type with an arbitrary user-specified dielectric function, although this is slower than using blocks for polygonal shapes.)
