I think that the problem can more easily be solved by you.  It is always
easier when someone else does it ;-)

This is actually a bug, because I do not think that the any space 
should be
created by \relax.  I may have inserted a space to avoid some other bug.
On the other hand, there is no "box" only the ascii character for a 
space
inserted into the RTF stream.

pdflatex would have failed as well.  '_' has a special meaning in latex 
and
therefore it has a special meaning in latex2rtf.

You should do this

\documentclass{amsart}
%use %latex2rtf: to redefine \bysame only in latex2rtf
%latex2rtf:\providecommand{\bysame}{\_\_\_\_\_\_}
\begin{document}
\nocite{*}
\bibliographystyle{amsplain}
\bibliography{publications}
\end{document}
