Well --extra-mem-top=n is as the documentation says
(http://docs.miktex.org/2.9/manual/pdftex.html) "relevant only after
the memory dump file has been read." 

But adding main_memory=5000000 to the local pdflatex.ini works fine
for me. I have no idea if you can change the value dest-names-size.
