Problems--XV and certain DVDs

I'm having a few problems with ogle on my Gentoo system.

First of all, fullscreeen doesn't actually make the video fullscreen. 
The FAQ says that this is a problem with xv not being set up right. 
Running xvinfo says that no adapters are present. However, running 
xvattr gives me "Found Xv 2.2" so it must be available somewhere. Do I 
need to add anything to my xorg.conf file or something?

Also, certain DVDs are unplayable. Most work fine, but one says "Could 
not open /dev/dvd with livdvdcss." This disc doesn't mount however, with 
mount giving the error "no medium found." Another one gives me a "Can't 
open file VIDEO_TS.IFO." but it mounts fine, and a file does exist at 
"/mnt/dvd/video_ts/video_ts.ifo" These are both discs in a series 
(different series from each other, though) and other discs in both 
series work fine. Any advice?
