Regarding "moderation queues" the comments plugin allows you to define what the extension of the saved comment is. If you set that to "cmt-", then you can moderate on the file system itself by searching your comments directories for files in "cmt-" and deleting the icky ones and renaming the ones you think are ok to "*.cmt". That's what I'm currently doing on my blog. All comments get moderated on my blog which is good because no spam gets through, but bad for all the obvious reasons.
