To me, a book is good to a person who wants it and/or will enjoy it if 
they get it.  Thus I can only speak for myself on saying mainstream 
publishing continues to be better at clearly labelling what a book is 
and delivering it to my proximity.  Some books are good to wide 
audiences, some to niches, and some are just not good to anyone but the 
author's mother.

I feel the strand of small press and self-publishing that rants against 
the mainstream can get a little like the vegetarian or gourmet (take 
your pick) telling the general public not to eat hamburgers because 
they taste terrible.  It reminds me of the regular rants the literati 
or more humorless feminist academics write against genre-romance.  
Ultimately the public know what they want and if presented with it, 
will buy it.  Most haven't any great notion or interest in where the 
book originated.
