You could try 'clear cache' from within akonadiconsole, after you shut 
down kmail.  Best you try this ONLY on ONE folder/subfolder, then check 
the situation within Kmail to make sure it worked.  Large folders may 
take some time.

If the above does not work, you can have a look at this process to 
recreate everything from scratch:
