Usually, when both services start and stay online together (because both
machines have rebooted simultaneously for example), you have a timeout
problem in piranha. Try to increase the timeout value to at least the
time it takes to postgresql to be available. Might solve you problem, at
the cost of slower failovers.

FOS is not part of the kernel. What exactly did you patch ?

Depends on where you got the patch from. As a general rule of thumb, I'd
say no. Too risky to assume a patch you've applied is on the next
release.
