New compromised machines are being created by worm/virus infections daily -- 
thousands of new ones every day. When  a spammer gathers up thousands of
'never before seen in spam headers' new zombies and does a huge spam blast
it takes all the blocklists time to catch up. This results in spams
"leaking" thru the filters. Short of whitelisting all your correspondents
and blacklisting everything else in your prefs, there is not much else that
you can do.

It certainly helps if you report the spams that make it thru your filter
defenses to help to ensure that the IPs get blocked.
