OK.  You can specify multiple parameters for the CPPFLAGS and LDFLAGS
using quotes.  Something like this might work (should be all one line
but thunderbird will probably wrap it, there is a space between db4 and
the bext -I):

./configure --with-db=/usr/local/lib CPPFLAGS="-I/usr/local/include/db4
- -I/usr/local/include" LDFLAGS="/usr/local/lib"
