I found the problem.  It was not Etherboot.

A bus analyzer revealed that this particular Phoenix TrustedCore BIOS 
was doing 2-byte ROM accesses, and our firmware wasn't handling some of 
them correctly.  It's fixed now, and the machine comes up into Etherboot 
from the .zrom.
