In both cases, the extra "+" at the  top of the tree indicates that the 
articles are both children (or later descendants), not parent and child. The 
graphical tree would display a downwards hook instead of "+".

In Case 2, the solution is to set new_subject_breaks_threads to 2 instead of 
0.
