If I recall correctly (while designing the input routines of FeederReader), I've seen in practice a <description> and <content:encoded> in the same <item>. When both are present, sometimes the <description> is a shortened version of the <content:encoded>. Often, this means the <description> is cut off after a certain number of characters and an elipsis (...) is placed at the end.
