Indeed, that has been the basis of much discussion at OttIX (Ottawa
Internet Exchange in Ottawa, Canada - www.ottix.net).  There had been
debate about pulling in the majors somehow, but the discussion is always
centred on questions like: "Why would they?" "What's the business case for
them?" "We're too small!"

Some of the thinking has been going the same way as Mike already said:
RFP's issued with the caveat that the bandwidth would be accessed via
OttIX, and that the winning bidder would have to deal with that.

So there are different ways to grow city or regional exchanges, and the
above is one way.
