Licensee name, contact details are?

What are the preferred values to use for this license: http://www.clickanduse.hmso.gov.uk/Online/CUP_Online_Application_Form.asp ? This will license us to use a wide range of UK government works.

I'm disinclined to enter details of any named individual - a role name seems like a better choice. This list is one email address possibility but a distinct mailbox which is set up now to forward to this list but can be pointed away from here if the volume increases is probably a better idea.

Do we have articles with a lit of licenses: applied for, denied, granted, so we can avoid repeatedly asking the same questions of people and organizations?
