A YubiKey 5C (Yubico 0x1050:0x0407, firmware 5.7.4) attached to fido(4)
stops answering CTAPHID requests after /dev/fido/0 has been opened and
closed a couple of times.  The device only recovers when it is physically
unplugged and plugged back in.

While a single file descriptor is kept open, the device works perfectly
and answers an unlimited number of transactions.  The failure is tied to
the open/close cycle, not to the number or type of commands issued.

Because both web browsers (WebAuthn) and ssh(1) open the device, use it
and close it again on every operation, the practical effect is that the
YubiKey works once or twice after being plugged in and is then dead: no
LED activity, no response, until it is re-plugged.

This worked on OpenBSD 7.7.  It broke on 7.8 and is still broken on 7.9.
The same key on the same machine works normally under Linux, so the key
itself is fine.

Observations that may help narrow it down:

- fido0 attaches normally and /dev/fido/0 can always be opened, even once
  the device has stopped answering.  open(2) succeeds, write(2) succeeds
  and reports 64 bytes written; only the reply never arrives.
- The report size handling is correct: a 64 byte write is accepted and a
  65 byte write is rejected with EMSGSIZE, so this is not a report ID
  framing problem in userland.
- After the failure, select(2) on the descriptor never becomes readable.
  A process blocked in read(2) stays blocked and holds the device, since
  fido(4) allows a single opener.
- Not related to pcscd(8).  This was my first suspicion, because pcscd
  holds ugen0.00, ugen0.02 and ugen0.03 open on the same physical device.
  Stopping and disabling pcscd does not fix the problem; it only removes
  one of the two open/close cycles the device survives, which made the
  failure look like it was caused by pcscd.

Every transaction on the single open descriptor succeeds, including the
WINK commands, which visibly blink the LED.  Once the descriptor has been
closed and reopened twice, the device never answers again until it is
physically re-plugged.
