Do you perhaps have to do a "boot -r" at the OK prompt? Or,
"reboot -- -r" at a root shell? Obviously, with the device attached.

Two other commands:

# disks
# tapes

I guess those are to add such devices on the fly, without a
reconfiguration reboot explained above.

As the other poster said, I'm assuming you have the correct
and proper drivers installed for your SCSI host adapter.
