fo:page-number-citation

	There are ways to generate a "real" word table of contents, i.e. by
adding the section header style and by inserting the RTF table of contents
keyword.  I also haven't yet found a way to automatically update fields upon
opening and the result looks real ugly before updating.  There is a update
before print option, but no update at load option.  A solution for updating
fields would allow us to use other Word fields.  I'd be very interested to
learn if we can find a solution for this.
